Welcome to our submission for the IChemE’s SIESO 2021 medal, helping to raise awareness of process safety among science, business and engineering students. We are a team from the University of Bradford, consisting of Lauren Averill, Michael Chu, Brady Durkin, Usaamah Ougradar and Alexander Reeves.
On the 20th of April 2010, a catastrophic oil well blowout occurred onboard the Deepwater Horizon Oil Rig which resulted in 11 deaths, the demise of the rig, and the largest oil spill in history. The events which led to the accident and the emergency response have since become a landmark case in process safety, law, and politics. Several important process safety management lessons were learned, ensuring the same mistakes never happen again.
